We realize that dropping your pet off for boarding can be stressful for many reasons.  You might be in a hurry or have a hard time saying goodbye.
While boarding, your pet will receive excellent care.  We learn the individual needs of all our boarders.  Some need quiet, some need extra walks, some need extra love.  We hope your pet enjoys their stay with us.  Filling out these forms will make the process quicker.  You may print the forms and bring them with you or just submit them from our website.  By submitting the form you are agreeing that your submitted form will serve as a signed form.  Your signed form is good for all boarding reservations in 2011.  Please remember that only an owner, co-owner or agent of owner can authorize services or pay for services for the pet.

For the safety of all animals visiting our hospital, proof of vaccinations is required.

I have been given a copy of Longenbaugh Veterinary Hospital's Boarding Policy and understand that I am responsible for all charges for services provided for my pet while boarding and that payment is due in full on the day of pick up from the hospital.
